Advance Care Planning
Advance care planning offers you the opportunity to plan your future care and support including medical treatment while you have the capacity to do so. It enables you to discuss and record your future health and care wishes.
It also allows the health care professionals and those closer to you to understand how you want to be cared if you become too unwell to decide or speak for yourself.
Remember advance care planning can make the difference between a future where a person makes their own decisions and a future where others do.
Not everyone will want to make an advance care plan, but it may be especially relevant for people at risk of losing mental capacity, have a serious or progressive illness, frail & elderly or those approaching end of life.
